What you'll do

Complete
6–9 in-home health assessments/day
Review medical history and risk factors
Collect vitals, labs, and screening data
Document findings in the EMR
Educate members on next steps
Coordinate follow-up if red flags are identified
Travel throughout

Active NP license
Master's in Nursing or higher
Valid driver's license and reliable vehicle
Able to work independently in the field
