Hearth Hospice is looking for a self-motivated and passionate Hospice RN Clinical Liaison to join our Team. The RN Clinical Liaison is primarily responsible for hospital and medical facility-based outreach to identify new referral sources and maintain ongoing relationships that will increase census and preference for hospice services.
