Physical Therapist (PT) - Home HealthSchedule: Full-time Monday–Friday Location: Missouri City and its surrounding areasCompensation: $75 per point Who We Are:Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health mode. Our ultimate mission is to dramatically improve patient access to home health care.The PT role is a core part of that mission: you restore function, keep patients safe at home, and drive real outcomes.We build technology designed around the way clinicians actually work, so you can spend less time on documentation and coordination, and more time with patients.Smarter charting: Record or dictate visits with AI Scribe, start with prefilled assessments, extract medication lists from photos, and keep all visit information in one easy-to-use place.Scheduling that protects your time: Text patients, plan more efficient routes, and stay aligned on visit changes through one clear, coordinated schedule.The result: less administrative burden, fewer coordination headaches, and more time for patient care. Responsibilities:Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.Complete comprehensive in-home assessments and deliver skilled PT <visits.Build> and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.Set measurable goals, track objective progress, and adjust treatment quickly when patients <stall.Drive> multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.Manage treatment and outcomes directly, including PTA supervision where <appropriate.Coach> patients and caregivers with practical home programs that actually get done.Document promptly and thoroughly Why AdaptiveOperations that back you up: You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.Clear productivity model with upside: You are paid per productivity point with an expectation of 28 points/weekSchedule flexibility: We build schedules to make field work sustainable, and we support smart routing and coordination, so your day is predictable whenever possible.Cutting-edge tech built for clinicians: Our charting tools are designed to reduce documentation burden, and our scheduling product will streamline visit coordination and reduce last-minute changes.Mission-driven work: We are here to expand access to high-quality home health care, and we want therapists who take pride in raising the standard.RequirementsMust-havesActive Texas PT license (in good standing)Strong clinical judgment and comfort working independently in the fieldComfortable escalating changes in condition to RN/MD and documenting rationale clearlyStrong communication and documentationReliable follow-through on orders, frequencies, and patient goalsCurrent driver’s license and comfort with daily travel across assigned territoryAble to carry/transport basic therapy equipment as neededCompetency in core home health PT scopeSafety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendationsStrengthening, functional mobility, and home safety assessment/modification educationPatient/caregiver coaching with practical home programsNice-to-havesPrior home health experienceEMR experienceDeeper clinical experience in high-signal PT populationsExperience supervising/mentoring PTAs and collaborating on care plansStrong organization and time managementBenefits401(k) + 401(k) matchingHealth, dental, vision, and life insurancePaid time offFlexible schedulingReferral programMileage Reimbursement
