DescriptionJob Summary: Performs basic duties of patient care to adult and geriatric populations, under the guidance and direction of the Registered Nurse. Performs various housekeeping and cleaning duties to maintain a safe, clean environment. Performance expectations are defined in the competency evaluation.

Job Responsibilities

• Admits/Discharges/Transfers patient and completes appropriate forms
• Provides for patient hygiene
• Provides for patients food and fluid needs
• Assists with patient mobility
• Vital signs
• Maintains safe environment of care
• Provides for appropriate pressure ulcer prevention
• Completes documentation accurately and in a timely manner according to hospital policies and procedures
• Provides age-appropriate and population specific patient care in a safe, skillful, accurate and efficient manner to primarily adult, and geriatrics populations as indicated
• Supports patient’s rights and responsibilities, including respect of cultural, ethical, and religious diversity.
• Adheres to standard precautions and infection control standards.

QualificationsEducation Requirements and Other Requirements

• Education Level: High school diploma or equivalent
• Certification/Licensure: Successful completion of approved Nurse Assistant course with certification by the State of Illinois
• BLS certification required
• Experience Requirements: Long term care, rehabilitation, and/or medical surgical care experience preferred
