You will serve as a resource for healthcare professionals by fostering relationships within medical facilities. With clinical expertise and problem-solving skills, you’ll also coordinate care plans with physicians and families. Additionally, you will facilitate patient transfers and address barriers to accessing specialized post-ICU care.
