Senior Care Management Peer Assistant

Helps guide team members in performance of duties. Serves as a functional mentor maximizing wellness, independence and provides peer support to behavioral health consumers. Collaborates with clinical and non-clinical team members to ensure culture of support is reflected in member's treatment.
