Comp: $17.00 - $18.00 per hour + mileage reimbursementSchedule: Full-time Monday–FridayLocation: Field-based- Missouri City podAdaptive Home Health is building a higher-acuity, patient-centered, skilled home health mode. Our ultimate mission is to dramatically improve patient access to home health care.The HHA role is essential to that mission: you provide compassionate personal care in the home, support patients in their daily activities, and help us raise the standard for what home health can be.We build technology designed around the way clinicians actually work, so you can spend less time on documentation and coordination, and more time with patients.Smarter charting: Record or dictate visits with AI Scribe, start with prefilled assessments, extract medication lists from photos, and keep all visit information in one easy-to-use place.Scheduling that protects your time: Text patients, plan more efficient routes, and stay aligned on visit changes through one clear, coordinated schedule.The result: less administrative burden, fewer coordination headaches, and more time for patient care.What you will do:Provide personal care assistance including bathing, dressing, grooming, and toileting.Assist patients with safe transfers, ambulation, and mobility using proper body <mechanics.Support> patients with medication reminders as directed by the care plan.Monitor and report changes in patient condition, vital signs (when ordered), and daily living activities to the supervising nurse.Perform light housekeeping tasks related to patient safety and care (laundry, meal preparation, maintaining clean environment).Document all care provided accurately and timely.Communicate clearly with patients, families, and the care team to ensure continuity of care.Follow infection control practices and maintain patient dignity and privacy.Assist with range of motion exercises and other restorative care activities as ordered.Coordinate with supervising RN/LVN for care plan updates and patient needs.Why AdaptiveOperations that back you up: You are supported by an experienced clinical and operational team so you can focus on patient care, not chasing logistics.Competitive hourly pay: $17–$18/hour based on experience, plus mileage reimbursement.Schedule flexibility: We build schedules to make field work sustainable, and we support smart routing and coordination so your day is predictable whenever possible.Cutting-edge tech built for caregivers: Our charting tools are designed to reduce documentation burden, and our scheduling product (launching soon) will streamline visit coordination and reduce last-minute changes.Mission-driven work: We are here to expand access to high-quality home health care, and we want caregivers who take pride in raising the standard.RequirementsMust-havesCurrent Texas CNA certification1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)CPR/BLS certificationCurrent Texas driver's license and reliable transportationAbility to lift, transfer, and provide hands-on care safelyStrong communication skills and professional demeanorComfortable working independently in patients' homesHigh school diploma or GEDNice-to-havesPrevious home health experienceFamiliarity with EMR experienceCNA or other clinical training/certificationExperience with medically complex or high-acuity patientsBilingual (English/Spanish)Care focus areasPersonal care and activities of daily living (ADLs)Mobility assistance and fall preventionVital signs monitoring and reportingCompanionship and emotional supportBenefits401(k) + 401(k) matchingHealth, dental, vision, and life insurancePaid time offFlexible schedulingMileage reimbursementReferral program
