About The Role

Liquid Personnel are currently recruiting for an experienced Occupational Therapist to join the Reablement Team within Brent Adult Social Care.

This is an excellent opportunity for an OT who enjoys community-based practice and supporting people to regain independence following hospital discharge or referral through community pathways.

You will work as part of a supportive multidisciplinary team, delivering strengths-based assessments and interventions that help residents achieve their goals, remain independent and reduce the need for ongoing support.

What will your responsibilities be?

Complete strengths-based Occupational Therapy assessments

Support residents following hospital discharge and community referrals

Carry out home visits across the borough

Assess functional needs and identify barriers to independence

Recommend appropriate equipment and adaptations

Develop person-centred intervention plans

Work collaboratively with

Social Workers

Care Assessors

Reablement Enablers

  • Health professionals
  • Hospital teams
  • Promote independence through rehabilitation and goal-focused support
  • Maintain accurate records and contribute to effective service delivery

Qualifications & Experience

To be successful in this role you must have,

HCPC registration as an Occupational Therapist

Experience working within community, social care or reablement settings

Experience completing functional assessments and recommending equipment/adaptations

Ability to manage a caseload effectively

Strong communication and MDT working skills

A person-centred and strengths-based approach

Confidence working independently within the community
