Social Worker - Learning Disabilities

About The Role

Liquid Personnel is recruiting for its client an experienced Social Worker to join a high-performing Learning Disabilities Team based in Louth, Lincolnshire, covering both rural and urban areas. The role involves supporting individuals to achieve positive outcomes through strengths-based assessments, care planning and reviews, including the use of Technology Enabled Care and community resources. You will manage complex cases, work collaboratively with partners, and ensure all practice meets relevant legislation while promoting independence and innovative support solutions.

What will your responsibilities be?

Undertaking assessments and reviews in line with relevant social care legislation

Managing a caseload within a Learning Disabilities service, including complex case work

Developing and updating person-centred care and support plans

Applying a strengths-based approach to assessments and care planning

Considering innovative solutions such as Technology Enabled Care (TEC) and non-statutory services

Promoting independence, inclusion and community engagement for individuals

Assessing and managing risk, ensuring safeguarding responsibilities are met

Working collaboratively with multi-agency partners, including health professionals and providers

Producing accurate, timely and high-quality case recordings and reports

Participating in multi-disciplinary meetings and contributing to decision-making processes

Making informed decisions regarding thresholds, referrals and support provision

Supporting the coordination and delivery of care packages

Ensuring all work complies with relevant legislation, policies and procedures

Using supervision effectively to reflect on practice and manage workload

Qualifications & Experience

To be successful in this role you must have,

Professional Qualification

MA, BA or BSc in Social Work, Diploma in Social Work, or CQSW

Registration

Must be registered with Social Work England

Experience

  • Minimum 2+ years’ post-qualified experience
  • Experience within Learning Disabilities and/or Adult Social Care
  • Strong experience in assessment, care planning and reviews
  • Experience of managing complex case work
  • Good legal literacy within adult social care
  • Ability to manage a busy caseload and prioritise work effectively
  • Experience working within a strengths-based, person-centred framework
  • Experience of multi-agency working and partnership collaboration
  • Evidence of ongoing professional development

Legislation KnowledgeStrong working knowledge of

Care Act 2014

Mental Capacity Act 2005 (MCA)

  • Human Rights legislation
  • Adult Safeguarding frameworks and procedures

Skills

Strong assessment and analytical skills

Excellent recording and report-writing ability

Ability to apply strengths-based and outcome-focused approaches

Excellent communication and interpersonal skills

Strong organisational and time management skills

Ability to manage complex workloads and competing priorities
