Social Worker - Preparation for Adulthood

About The Role

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Preparation for Adulthood team, based in Sheffield.

What will your responsibilities be?

Transition Planning — Lead holistic, person‑centred transition assessments for young people (14–25) moving from children’s to adults’ services, ensuring Care Act eligibility, strengths‑based planning, and coordinated multi‑agency pathways.

Risk & Safeguarding — Manage complex cases involving safeguarding concerns, capacity issues, or escalating needs, providing professional oversight, Mental Capacity Act application, and robust risk management.

Multi‑Agency Coordination — Work closely with education, health, SEND, commissioning, and community partners to secure appropriate adult‑life outcomes, including housing, employment, independence skills, and ongoing support packages.

Qualifications and Experience

To be successful in this role you must have;

Social Work England registration

Eligible to work in the UK

Hold a full UK driving licence

Post-qualified experience

Degree level or equivalent in Social Work
