The Case Manager coordinates all services necessary to maxim

e consumer recovery and independence for individuals with mental illness including implementing strategies for obtaining services and supports that are goal-oriented and individualized.
What You’ll Do...
Case Management
- Ensure consumer involvement in the Person Centered Planning Process (PCP).
- Facilitate the implementation of Individual Plan of Service.
- Perform comprehensive, ongoing psychosocial assessments.
- Participate in legal proceedings including petitioning and testifying in clinical emergencies.
- Provide community based services.
- Facilitate the coordination of consumer intakes, discharges, and transfers.
- Communicate with all service providers in Treatment Plan.
- Facilitate hospitalizations, attend hospital meetings and coordinate with hospital social worker.
- Coordinate and monitor activities such as a job, school, volunteerism, and family caretaking.
- Provide referral to appropriate community resources.
- Please note…
- The CRU Case Manager position is weekdays and weekends. It typically includes every other Saturday and Sunday.
- The 2nd shift CRU Case Manager would work 4p-12a until they are medication trained then move to the 3p-11p slot.
- CRU Case managers are not on call. However they do assist if the Program Manager is on PTO.

- What you’ll bring...
Required
- Bachelor’s degree in a human services field from an accredited institution.
- Valid Michigan Driver’s License.
- Willingness to work in the community and perform home visits.
Preferred
LLBSW, LBSW, LLMSW, LMSW, LMFT
Prior case management experience in a mental health or rehabilitation setting preferred.
