Position Summary

The Care Coordinator plays a pivotal role in organizing, streamlining, and monitoring patient care plans and day-to-day healthcare operations. Serving as the primary point of contact between patients, families, healthcare providers, and administrative staff, this position ensures seamless communication, timely service delivery, and high standards of patient care.

Key Responsibilities

Patient & Provider Coordination

Act as the central hub of communication between patients, physicians, specialists, and multidisciplinary care teams to ensure coordinated delivery of services.

Care Plan Management

Assist in building, updating, and monitoring individual patient care plans, tracking progress against clinical goals, and scheduling necessary follow-up appointments.

Administrative & Workflow Support

Maintain accurate electronic health records (EHR/EMR), process referrals, perform insurance eligibility verification, and handle incoming patient inquiries via phone, email, or chat.

Resource Facilitation

Connect patients with community resources, home care services, medical equipment providers, and financial support programs as needed.

Patient Advocacy & Outreach

Conduct routine check-ins with patients to monitor treatment adherence, address care barriers, and educate patients on their prescribed care plans.

Quality Assurance & Compliance

Ensure all coordination activities strictly adhere to HIPAA regulations, patient privacy laws, and standard operating procedures (SOPs).

Issue Escalation

Identify delays in treatment, care gaps, or recurring operational issues and flag them promptly to clinical leadership for resolution.

Required Qualifications

Experience

3+ years of experience in healthcare coordination, medical administrative support, patient navigation, or case management.

Technical Skills

Proficiency with Electronic Health Record (EHR) systems, HIPAA-compliant communication tools, and general office software (Microsoft Office Suite, Google Workspace).

Communication

Exceptional verbal and written communication skills with strong empathetic listening abilities.

Organization

Outstanding time-management skills, high attention to detail, and a proven ability to manage multiple care tracks simultaneously.

Preferred Qualifications

  • Prior experience working in a remote healthcare or telehealth environment.
  • Medical billing, coding, or insurance verification knowledge.
  • Bilingual capabilities (language requirements as needed by your patient demographic).
