Care Manager

to join our

Health Home Serving Children (HHSC)

program—supporting youth with complex needs to achieve wellness, stability, and success.

šŸ’¼ About the Role

  • As an HHSC Care Manager, you'll serve as the
  • primary point of contact
  • for children with chronic conditions or behavioral health challenges and their families. You’ll coordinate care across providers, schools, and support systems to ensure a
  • comprehensive, child-centered plan of care
  • is in place and followed.
  • Your work will span

Brooklyn and Queens

, meeting children and families where they are—at home, in the community, or virtually.

šŸ”‘ Key Responsibilities

  • Conduct comprehensive assessments and develop individualized care plans
  • Coordinate medical, behavioral health, educational, and social services
  • Serve as a liaison between families, providers, schools, and support systems
  • Track and document progress toward health, social needs, and wellness goals
  • Coordinate transitional care from Emergency Room/Inpatient Admissions to ensure safe discharge to the community
  • Advocate for children’s needs while empowering families
  • Attend regular supervisory and case conference sessions, trainings and other meetings, as assigned.

  • Passionate about child welfare, behavioral health, and equity in healthcare
  • Skilled in communication, cultural sensitivity, and problem-solving
  • Organized, proactive, and able to work independently and collaboratively
  • Comfortable navigating Brooklyn/Queens neighborhoods for field visits

āœ… Qualifications

  • Bachelor’s degree
  • in social work, psychology, sociology, education, nursing, public health, or a related field
  • At least
  • 2 years of relevant experience
  • working with children/families
  • Knowledge of Medicaid, Public Assistance, SNAP, and other and community-based services a plus
  • Valid NYS Driver’s License or reliable transportation for community visits
