Patient Care Coordinator

Patient Care Coordinator – MedSleep, Ontario MedSleep provides clinical consultation, sleep testing, and treatment for sleep disorders across Canada. We're growing our team and looking for a Patient Care Coordinator to join a MedSleep clinic in Ontario. What You'll Do Act as the main point of contact between patients, physicians, and clinical staff throughout the patient's care journey Review assigned tasks and patient information, then contact patients to discuss next steps Update patient charts and coordinate with referring physicians and other healthcare providers Email patients with required information and next steps Help patients navigate the process – answering questions and connecting them to the right person when needed What We're Looking For 1+ year of administrative experience (required) Knowledge of sleep medicine or CPAP therapy (an asset) Sales experience (an asset) Strong organizational skills and attention to detail Ability to adapt quickly to new procedures Strong verbal communication and time management skills Ability to multi-task and prioritize under pressure Strong listening and direction-following skills What We Offer Competitive salary ($18–20/hour) Comprehensive benefits package Group RRSP plan Opportunities for career growth and professional development Direct patient interaction and meaningful, hands-on work Job Details Full-time, permanent 37.5 hours/week Location: local MedSleep clinic, Ontario – in-person
