Bilingual Care Team Assistant - Job Overview

Schedule: Monday-Friday (8:30AM-5PM)

Key Responsibilities

Patient Communication

Work with onsite nursing or follow protocol-based triage to address patient phone calls and route clinical inquiries to providers. Return calls and communicate results as directed.

Pre-Visit Planning

Review patient schedules daily, ensure lab results and consultation reports are available, and gather necessary information. Assist in arranging interpretation services and participate in daily huddles.

Care Coordination

Manage transitions of care, support access to ancillary services, and maintain the Care Coordination Desktop.

Medication Management

Assist with medication refills and prior-authorizations.

Forms

Initiate and complete forms for review and final signature by the provider. Address urgent requests promptly.

Referrals

Complete referrals, deliver clinical summaries for consultants, and ensure relevant documents are included in patient records.

Safety and Compliance

Promote effective working relations to facilitate goal achievement and adhere to safety training and equipment operation.

Minimum Qualifications

  • High-school level education or equivalency
  • Medical Assistant Certification or its equivalent, or at least 2 years clinical experience in an inpatient or outpatient setting
  • Must maintain BLS /CPR
  • At least 2 years of experience in an ambulatory clinical setting preferred
  • Spanish fluency
  • Demonstrated competency with an EHRS system
  • Excellent communication skills.
  • Excellent customer service skills.
  • Ability to multi task in a fast paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills.
  • This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management
  • Must be able to manage multiple deadline and decision making responsibilities.
