About The Role

The Hospital Access Liaison provides rapid, compassionate support to individuals recently discharged from the Emergency Department or inpatient units. This role focuses on linking high-risk clients to community and behavioral health services, supporting continuity of care, and promoting recovery and wellness.Full-time, benefit eligible position. The schedule is Monday - Friday 8:00am-4:30pm. This is a great opportunity for someone who thrives in fast-paced, collaborative environments and wants to make a meaningful difference in the lives of individuals transitioning from hospital care back to the community.
