OPEN POSITION: Nurse Practitioner - Geriatric Primary Care - Skilled Nursing Facility SCHEDULE: - Full-Time - 4-Day or 5-Day Schedule - No Evenings - No Weekends - No Holidays COMPENSATION: - $125,000 to $170,000 Annual Income - $105,000 to $125,000 Base Salary - $20,000 to $45,000 Annual Bonus Income, paid quarterly - Malpractice Insurance - Health / Dental / Vision Insurance - AD&D / STD / LTD Insurance - Paid Time Off - CME Allowance - License / DEA Fees Paid - 401k LOCATION: Mentor, Ohio COMPANY PROFILE: This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 30 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, psychiatric care, post-acute care, and transitional care. POSITION DESCRIPTION: The nurse practitioner will serve as the primary care provider for geriatric patients located within a single skilled nursing facility (SNF) in Mentor. Providers see a variable number of patients per day, dependent upon patient needs. Specific duties of the role include, but are not limited to: - conduct health assessments to identify patient needs - develop personalized care plans - regularly evaluate patient conditions and responses to treatment - adjust treatment plans as necessary - perform minor procedures and wound care as needed - educate patients and families on health management, disease prevention, etc. - provide guidance on lifestyle modifications and self-care practices - work with physicians, nurses, and other professionals to coordinate care The practice uses a mobile-based EHR with AI functionality to reduce administrative time. On-call is phone triage only and providers rotate the responsibility evenly, with some flexibility in scheduling available.
