Medical Social Worker (Homecare)

• Explore and provide psycho-social/emotional care and support to patients and caregivers/family.
• Lead in ACP discussions and completion. Complete Preferred Care Plan
• Provide Bereavement Support.
• Coordinate in-house Financial Assistance for needy patients.
• Coordinate Medical Escort & Transport for patients.
• Coordinate in referring patients for Respite Care, Nursing Home and other necessities.
• Counselling/ Reconciliation
• Crisis Intervention
• Education, Teaching & Networking

Job Requirements

• Degree or postgraduate diploma in Social Work
• Preferably 2 years relevant experience
• Must have interest to work with elderly
• Displays compassion, a positive and caring attitude towards patients, internal and external clients.
• Excellent communication, interpersonal and problem solving skills
• Meticulous, diligent and a good team player.
• Adaptable and comfortable with changes
