Qualifications

Minimum

• 1 year of recent LPN experience
• (acute care required; home care preferred)
• Active New Jersey LPN license

and

• current BLS certification
• Authorized to work in the United States
• Current physical exam
• (within the past 12 months)

PPD or QuantiFERON

(within 12 months)

or

• chest X-ray (within 5 years)
• MMR immunizations
• (within 10 years)
• Flu vaccination
• or approved medical exemption
• Valid driver’s license (preferred)
