Job Purpose
The Registered Nurse at Parentis Home Health is responsible for providing high-quality patient care through comprehensive assessments, planning, implementation, and evaluation of nursing care within a home environment. The role requires collaboration with healthcare team members to ensure optimal patient outcomes and adherence to established medical protocols and standards.
Key Responsibilities
- Conduct thorough patient assessments to identify healthcare needs and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians, ensuring accuracy and compliance with healthcare regulations.
- Monitor and document patient progress, adjusting care plans as necessary to achieve desired health outcomes.
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care and ensure seamless service delivery.
- Educate patients and their families about health conditions, treatment plans, and preventive measures to promote wellness and recovery.
- Maintain accurate and timely patient records, ensuring confidentiality and compliance with healthcare regulations.
- Participate in continuous professional development and training to stay current with nursing practices and advancements.
- Uphold Parentis Home Health's commitment to excellence in patient care by adhering to ethical standards and organizational policies.
Qualifications
Required Education
- Bachelors degree in Nursing
Required Experience
- Minimum of 2 years of clinical experience in a home healthcare setting.
- Experience in patient care management and coordination.
- Familiarity with electronic medical records and healthcare information systems.
Required Skills and Abilities
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team members.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficiency in administering medications and performing medical procedures.
- Strong organizational skills with the ability to manage multiple priorities and tasks.
