Pharmacist (Inpatient Pharmacy)

• Provide comprehensive pharmaceutical care and services for inpatient patients in accordance with hospital standards, pharmaceutical practice standards, patient safety requirements, and applicable laws and regulations.
• Review and validate medication orders and prescriptions, including medication appropriateness, dosage, route of administration, drug interactions, allergies, contraindications, and potential medication-related problems, and coordinate with physicians and other healthcare professionals when required.
• Prepare, compound, dispense, and distribute medications and pharmaceutical supplies accurately and safely according to medication orders, including ensuring proper labeling, storage, handling, and timely delivery of medications to inpatient units.
• Provide medication information and education to patients and families as needed, and collaborate with doctors, nurses, and other multidisciplinary teams in monitoring medication therapy, identifying adverse drug reactions, and optimizing patient treatment.
• Maintain complete and accurate pharmaceutical documentation, medication records, and reports, while implementing medication safety, infection prevention, quality improvement, inventory control, and other procedures to support hospital-wide and department/unit targets.
• Minimum Bachelor’s Degree in Pharmacy (S.Farm.) and Pharmacist Professional Program (Apt.)
• Must hold an active Pharmacist Registration Certificate (STR)
• Have good knowledge and competency in prescription review, medication dispensing, pharmaceutical care, medication counseling, and medication safety
