Pharmacist (Outpatient Pharmacy)

• Provide comprehensive pharmaceutical care and services for outpatient patients in accordance with hospital standards, pharmacy practice standards, applicable regulations, and patient safety requirements, including prescription review and identification of medication-related issues.
• Prepare, dispense, and ensure the availability of medicines and pharmaceutical supplies accurately and safely, including proper medication preparation, labeling, storage, inventory monitoring, and expiry date control.
• Provide medication counseling and education to patients regarding medication use, dosage, administration instructions, potential side effects, storage, adherence, and other relevant information to promote safe and effective medication therapy.
• Collaborate with doctors, nurses, and other multidisciplinary healthcare teams in optimizing medication therapy, resolving prescription or medication-related issues, and ensuring continuity and quality of patient care.
• Maintain accurate pharmaceutical documentation and reporting, implement medication safety and infection prevention practices, and participate in quality improvement initiatives, mandatory training, competency development, and continuing professional education.
• Minimum Bachelor’s Degree in Pharmacy (S.Farm.) and Pharmacist Professional Program (Apt.)
• Must hold an active Pharmacist Registration Certificate (STR)
• Have good knowledge and competency in prescription review, medication dispensing, pharmaceutical care, medication counseling, and medication safety
