Job Summary
The Clinical Team Leader is responsible for oversight of one or more clinical teams. Reporting to either the Chief Clinical Officer or the Director of Access & Palliative Care, the Clinical Team Leader works under limited supervision to oversee their assigned clinical area. *This vacancy will manage a home hospice team and will split time between the Healthway Office in Salisbury, Maryland and the Macky & Pam Stansell House in Ocean Pines, Maryland.*
Essential Functions
- Participation in management team and assists in administration
- Ensures a consistent high quality hospice experience for each patient and family by facilitating comprehensive patient care planning and monitoring performance of individual team members
- Supervises interdisciplinary team (IDT) members operationally and administratively, facilitating efficient and effective delivery of service
- Leads IDT meetings and facilities care planning by ensuring opportunity for all disciplines to engage in the care planning process; oversees documentation of the IDT meeting
- Confers with appropriate members of the IDT regarding referrals, changes in patient level of care, contemplated discharges, patient hospitalizations, and any other significant patient changes in a timely manner
- Recruits, interviews, hires, and trains staff members in collaboration with discipline-specific clinical experts and the Education Coordinator
- Conducts employee performance reviews timely and effectively
- Conducts investigations, counsels, and takes appropriate corrective action in collaboration with Human Resources
- Assures nursing documentation is complete and timely in accordance with legal and regulatory standards
- Identifies learning needs of staff and provides educational resources to meet employee needs; serves as a resource and mentor to staff
- Cooperates with other leaders throughout the agency to assure program needs are met and resources are used effectively; identifies and involves all stakeholders in the decision-making process and communicates proactively
- Ensures department financial goals are met by managing to the Coastal Hospice model
- Ensures department and organizational QAPI goals are met
- Driver’s License in your state of residence with a good driving record and access to a reliable automobile
- Must possess automobile insurance coverage with minimum limits of $100,000/$300,000
Duties/Responsibilities
- Shares in responsibility for management/leadership on call 24/7
- Accountable for meeting patient care needs 24/7 via appropriate scheduling and delegation to members of the IDT
- Participates in the development and implementation of protocols, policies, and procedures related to patient care and organizational goals
- Performs other duties as assigned
- *Access Services Team Leader:*
- Manages access services nurses, liaisons, and coordinators
- Reports to the Director of Access & Palliative Care
- Collaborates with inpatient and home care teams
- Conducts admissions assessments, as needed
- *Extended Services Team Leader:*
- Manages home hospice staff to complete home and hospital visits as appropriate and timely
- Oversees triage phone calls from patients and families needing assistance when the office is closed
- Provides medical care to patients in their home environment, as needed
- Conducts admissions assessments, as needed
- *Home Care Team Leader:*
- Manages team staff, including RNs, LPNs, and CNAs
- Assures smooth functioning by:
- Schedules appropriate nursing and CNA staffing levels to meet patient needs
- Maintains effective professional relationships with hospitals, physicians, and other health care providers
- Assures documentation of IDT members is complete and timely in accordance with legal and regulatory standards
- Builds and maintains relationship with facilities in team’s area
- Provides medical care to patients in their home environment, as needed
- *Inpatient Unit Team Leader:*
- Manages inpatient unit staff, including RNs, LPNs, and CNAs
- Assures smooth unit functioning by:
- Scheduling appropriate nursing and CNA staffing levels to meet unit needs
- Oversight of adequate inventory of medical supplies, equipment, and linens
- Maintaining effective professional relationships with hospitals, physicians, and other health care providers
- Assuring documentation of IDT members is complete and timely in accordance with legal and regulatory standards
- Maintaining a safe physical environment according to unit needs and regulatory standards
- Oversees daily bed management to ensure optimal patient flow and timely admissions
- Coordinates with referral sources, Access Team, and Home Care teams to assess bed availability and prioritize patient placement
- Monitors census levels and proactively anticipates discharges, transfers, and new admission needs
- Reviews admission criteria and supports decision making to ensure appropriate and safe patient placement
- Identifies barriers to bed availability and participates in process improvement initiatives to enhance throughput
- Provides care to hospice patients in an inpatient setting, as needed
- Macky & Pam Stansell House only:
- Manages all Coastal Hospice social workers
- Serves as a resource and mentor to inpatient staff and social workers
- Acts as Ethics Committee Chair for the organization
Supervisory Responsibilities
- Responsible for oversight of assigned team, which may include Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and/or Social Workers
Qualifications
Required Skills/Abilities
- Two years of management experience
- Excellent communication skills
Required Education and Experience
- Registered Nurse from the State of Maryland or participating Compact state OR licensed LCSW-C in the State of Maryland (Macky & Pam Stansell House only)
- Certified Hospice and Palliative Nurse (CHPN) or attainment within two years for RNs
- Advanced Certified Hospice & Palliative Care (ACHP-SW) or attainment within two years for Social Workers
Preferred Education and Experience
- Previous hospice experience
Physical Requirements
- Ability to safely operate an automobile
- Ability to lift and carry up to 50 pounds with or without the use of assistive devices
- Ability to use hands and fingers for repetitive motions like typing and using a mouse
- Close visual acuity for reading small print and using a computer
