Registered Nurse/RN - Home Health Admission

The Admission RN initiates all aspects of the patient’s home health care by performing the start of care assessment visit. He or she determines patient eligibility for care, completes most OASIS assessments, and develops evidenced-based individualized care plans following established professional standards of care, Agency policies, and State, Federal laws and regulations. The Admission RN is integral to optimal quality and process outcomes. This position works 7 consecutive days and is off 7 consecutive days.
