This assignment requires Vermont OT licensure (may be pending) and BLS certification. Key Highlights - Weekly pay: $2,355–$2,393 per week - Contract duration: 16 weeks - Shift: 5x8 days (8-hour days; 40 hours per week) - Start date: 10/12/2026 - Setting: Long-term care nursing home - Prior travel experience required Responsibilities - Perform comprehensive OT evaluations for residents in a long-term care setting. - Develop and implement individualized treatment plans addressing activities of daily living, mobility, cognition, and safety. - Monitor progress, document outcomes, and adjust plans in collaboration with the interdisciplinary team. - Provide patient and family education on therapeutic exercises, adaptive equipment, and safety strategies. - Maintain accurate records in the facility’s EMR and participate in care planning meetings with nursing and medical staff. - Attend bi-weekly orientations on Mondays and comply with site onboarding and policies as required.
