Patient Enrollment Coordinator

The Patient Enrollment Coordinator works alongside Discharge Planners, facility leadership, and local county agencies to support Medi-Cal coverage verification, enrollment activities, and Justice-Involved Reentry Initiative processes for individuals in custody. This role verifies existing Medi-Cal coverage, Client Identification Numbers, and managed care plan assignments and assists with enrollment activities when coverage is not active. The position coordinates with county agencies regarding enrollment determinations and manages applicable Justice-Involved aid code activation and deactivation through the JI Reentry Portal. Eligibility and enrollment determinations are made by the appropriate county social services or welfare agency.
