Are you an experienced public health nurse passionate about preventing disease, expanding access to care, and supporting healthier communities? Join Oakland County’s Health Division at the Pontiac Clinic and provide essential nursing services to individuals and families throughout the community.

What You’ll Do

• Provide experienced professional nursing care and consultation services at the Pontiac Clinic and in community-based settings.
• Assess health needs and provide family-centered care focused on disease prevention, early detection, treatment support, and appropriate referrals.
• Administer vaccinations and maintain the clinical skills necessary to support immunization services.
• Conduct follow-up investigations and coordinate care with healthcare professionals, schools, community agencies, and other partners.
• Educate and counsel individuals, families, and groups using preventive, therapeutic, and rehabilitative techniques.
• Promote community health through outreach services provided at schools, childcare centers, businesses, community centers, senior living facilities, homes, and community clinics.
• Independently plan and implement public health nursing services within assigned program areas.
• Collaborate with other professionals to address known or suspected health concerns affecting individuals and families.
• Respond to disease outbreaks, high-volume community outreach events, and other emerging public health nursing needs.
• Maintain accurate records and documentation using countywide and department-specific software.

Minimum Qualifications

EXPERIENCE, TRAINING, KNOWLEDGE, SKILLS & ABILITIES

REQUIRED MINIMUM QUALIFICATIONS

1. Be licensed by the Michigan Board of Nursing to practice as a registered professional nurse in the State of Michigan.

NOTE: A Bachelor's Degree in Nursing from an accredited college or university is preferred.

NOTE: Applicants who are eligible to be licensed by the State of Michigan and who meet the other requirements listed above may take this examination but will not be eligible for appointment until they have been licensed by or possess a valid permit from the Michigan Board of Nursing.

ADDITIONAL DESIRABLE QUALIFICATIONS

• Knowledge of modern principles, methods, materials, practices, and specialized terminology of public health professional nursing service.
• Ability to apply the principles, practices, and techniques of professional nursing to the functioning of Health Department Clinics, and services programs offered to schools, nursing homes, day care centers, individual homes, or other settings.
• Ability to establish and maintain effective working relationships with Health Division professional and support staff, and the public.
• Ability to cope with difficult and/or emergency situations requiring immediate decisions in accordance with Health Division policies and procedures.
• Ability to compile pertinent information and prepare and present oral or written technical reports to supervisors, staff, and the public.
• Ability to exercise mature judgment and initiative in analyzing problems and recommending solutions.

SPECIAL REQUIREMENTS

• Submit to immunizations as required by the Health Division and to tuberculin skin tests or chest X‑rays at County expense.
• Maintain current registration with the State of Michigan Board of Nursing.

ESSENTIAL JOB FUNCTIONS

• Provides nursing care services to clinic population, schools, nurseries and day care centers such as immunizations, child health conferences, the identification, reporting and control of communicable diseases, health education, nutrition and counseling. Performs follow-up of health problems detected by physicians, hospitals and clinics.
• Acts as a resource for teachers, students and parents in health-related problems. Assists schools, nurseries and day care centers in establishing first aid systems and health safety procedures. Works with administrators, educators and physicians in assessing health and immunization needs and activities to meet these needs. Makes appropriate referrals to Health Division programs or outside health care agencies.
• Provides care and consultation services to county residents utilizing clinical services or in assigned geographical areas through home visits to individuals or families. Teaches, instructs and demonstrates preventive, restorative and supportive health education to individuals referred to from hospitals, clinics and community agencies.
• Provides nursing services at various outreach clinics, community centers or Health Division clinics, such as assessing health needs of infants and children through child health conferences, giving immunizations to children through Project Head Start or Medicaid clinics. Performs counseling and referrals for individuals who call or visit Health Division clinics.
• Provides screening and diagnostic services to the adult population through programs dealing with tuberculosis, venereal disease, breast cancer and hypertension.
• Works with schools, community health care and social services agencies in planning, developing and coordinating health education activities to ensure continuity of patient care, planning and exchange of information. Participates in planning committees to provide Health Division program input. Works with State Health Department representatives to communicate community input for statewide planning.
• Teaches classes for children and adults such as hypertension, expectant parent, teen mothers, diabetics, and restorative nursing for the aging. Develops educational materials, instructs, counsels and acts as a resource for individuals, groups and Public Health Nurses.
• Provides observation and/or role modeling for baccalaureate nursing students and other allied health professionals.
• Acts as a liaison with school district administration, health care agencies, hospitals and volunteers in developing policies and procedures for comprehensive health treatment. Acts as a resource for these agencies and as a representative of the Oakland County Health Division to these agencies in terms of nursing and health activities. Works with health care and medical professionals in diagnosis of disease/health conditions and treatment alternatives.
• Meets with supervisors to plan, coordinate and evaluate nursing care activities and to assess individual client needs. Discusses policies, procedures and provides information to supervisors for evaluation of nursing service. Provides leadership on assigned Health Division agency task force to assist in assessment of community health needs.
• Performs record keeping activities related to clinical services, schools and home visits. Codes agency information for computer input of client services, documents work activities and plans schedule of clinical services and schools and home visits. Develops performance standards for professional skills building.
• All nurses will be required to respond to outbreaks, high volume community outreaches, and/or other emerging nursing needs.
• Maintains necessary clinical skills needed to administer vaccinations.
• May be required to travel to multiple worksites within a single day.

ESSENTIAL MENTAL & PHYSICAL REQUIREMENTS

• Ability to communicate to accurately convey information.
• Ability to use and operate medical office equipment and supplies.

WORKING CONDITIONS

• Work is performed in a typical medical clinic environment as well as in patient homes, and educational or institutional settings.
