  • Up to a $10,000 Sign on Bonus Available for eligible applicants

Shift : Monday- Friday 8am-5pm with Saturday coverage every 5th weekend.

Location: Coverage area of Whitfield and Murray Counties.

Develops professional and clinical skills through attendance at staff development meetings, educational activities, and professional affiliations. Admits new patients according to departmental policy and performs comprehensive initial evaluations for those requiring physical therapy. Conducts home safety evaluations and communicates specific needs to the physician and appropriate team members. Establishes a plan of care with goals based on the patient’s current level of functioning and potential for improvement, in conjunction with the patient, family, caregiver, and physician’s orders. Implements the physical therapy assessment and care plan, continuously evaluating the patient’s response and modifying as needed. Reports pertinent observations to the attending physician and adjusts the patient care plan as directed. Instructs patients and families in the physical therapy program, including the use of prosthetic, orthotic, and assistive devices. Participates in the team process, through cooperative efforts with team members, in developing the patient care plan and attending team conferences. Supervises and plans home health aide activities in the absence of other healthcare professionals, according to individual patient care requirements and department policy. Demonstrates effective communication skills with all team members, patients, and families. Maintains clinical record information in accordance with established standards and within established time frames. Other duties as assigned. Participates in the review process, agency performance improvement program, orientation, in-service, and staff meetings as scheduled. Displays flexibility in accepting assignment changes and patient care responsibilities, including participation in weekend coverage and call rotation.

  • Knowledge and competency to perform required documentation and patient care procedures as a PT and to effectively manage various demands of the job.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Requires individuals to be able to make emergency and mature independent judgments and decisions.
  • Knowledge in the identification, prevention, remediation, and rehabilitation of patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other causes.
  • Demonstrates flexibility and is able to interact positively with associates.

Pay Range

  • $33.50 - $62.31
