About The Role

INTERNAL JOB POSTING - ONLY INTERNAL APPLICANTS WILL BE CONSIDERED Position Title: Community Action for Prenatal & Child Health Program (CAPCHP) Full Time Permanent Hours of Work: Flexible (35 hours per week) Location: Golf Links R eports to: Program Manager or designate Salary: Schedule A Salary Grid Appointment: Full Time Affiliation: Union – Unifor Local 229 Police Records: Vulnerable Sector Position Summary The CAPCHP is responsible for managing the daily operations of the Community Action Plan for Prenatal Child Health Program (CAPCHP) within an Anishinabe Health and Wellness Framework Ke y Responsibilities/Duties Work from a holistic perspective, utilizing the teachings of the medicine wheel and Seven Grandfather Teachings. Utilize Elders for teachings, healing and guidance. Take personal responsibility to increase sensitivity, awareness and implementation of Beendigen’s cultural teachings and organizational practices in both professional conduct and work-related deliverables. Coordinate project activities in conjunction with existing community programs and facilities that meet Program goals and objectives. Program Coordination & Delivery Plan, implement, and evaluate prenatal, postnatal, and early childhood programs Deliver prenatal education, nutrition counselling, breastfeeding support, parenting programs, and prevention programming Provide direct support through home visits, hospital visits, and community-based programming Coordinate program activities with community services and facilities Case Management & Family Support Provide individual and group support to families Support high-risk population Assist families in achieving personal and wellness goals Provide referrals and system navigation Cultural & Community Integration Deliver services using the Medicine Wheel and Seven Grandfather Teachings Engage Elders for guidance and healing Build and maintain community partnerships Promote culturally grounded programming Program Administration & Reporting Maintain program statistics and reports Ensure compliance with funding agreements Develop proposals and program policies Operations & Logistics Ensure availability of materials and equipment Arrange transportation for participants Maintain safe program environments Organizational Responsibilities Adhere to policies and procedures Maintain confidentiality Participate in meetings and training Promote the organization Public Relations: Promotes Beendigen at all times, when networking and events. To adhere to professionalism in always representing the organization. Maintain excellent communication and community connections. Expected Standards of Performance Adhere to the Policies and Procedures as set by Beendigen. Models expected behaviors as outlines in the Policies and Procedures of Beendigen. Representation on committees as required for the enhancement and benefits of Beendigens programs. Always maintain professionalism. Adherence to the Code of Ethics and positive relationship building. The completion of functions outlined in the description and the achievement of goals set to a high level. Regular attendance and conduct at work according to the requirements of the Beendigens Policies and Procedures. other duties Any other duties as requested by the Program Manager or designate, commensurate with role. Key Performance Indicators That the programs are regularly evaluated for continuous improvement. That membership of the Leadership Team contributes to the implementation of the organization’s strategic plan. Personal responsibility has been taken to increase sensitivity and awareness of the organization’s Anishinabe Health and Wellness Framework. That programs and services are culturally based and incorporate traditional knowledge and practices of healing and wellness. The number of adults and children attending programs through the year. Services provided meet expectations of clients. Evidence of partnerships with external organizations. That annual evaluations are completed and filed with head office and a copy is provided to the staff. Enhanced skill level through training, learning and development. Qualifications Diploma or Degree in Social Work, Nursing, Health Sciences, or Human Services. Experience in prenatal/postnatal care and/or child development. Knowledge of Indigenous health issues and strong organizational and communication skills. Preferred Experience working in an Indigenous organization. Ability to speak Ojibway, Oji-Cree or Cree. Accountability The CAPCHP worker will be accountable to the Program Manager or designate for the proper completion of the functions outlined in the job description. Working Conditions CAPCHP worker usually work in group settings and an office, but the mission of the organization may sometimes take them to non-standard workplaces. The CAPCHP worker works a flexible 35-hour work week; Monday to Friday. Ontario Health & Safety (OH&S) All employees are required to carry out their duties in a manner that does not adversely affect their own health and safety and that of other by reporting all potential hazards; and all incidents and injuries; as well as co-operating with any measure introduced in the workplace to improve OH&S.
