About The Role
Position Summary The Home Care Intake Specialist supports the home care admission process by partnering with Home Care Transition RNs to coordinate referrals, enter referral information into the electronic medical record (EMR), and ensure patients are scheduled for timely Start of Care. This role serves as a key point of contact for hospitals, physician offices, and other referral sources while providing exceptional customer service and ensuring accurate, efficient referral processing.
