About The Role
Starting Rate: $21.00/hour Schedule: Weekdays (Flexible Hours) The Case Manager is responsible for documentation and maintenance of program participants’ records and plans, including all areas of the Individual Service Plan (ISP), medical concerns and appointments. This role will focus on individual advocacy, empowerment, and community integration.
