Patient Flow Coordinator Per Diem

Identify and prioritize patient admissions through admission records and within the hospital via recovery room, emergency department, etc., records. Assign patients to appropriate floor, bed type, and status based on criteria identified upon admission or physician orders. Notify the receiving units of the patient admission particulars and bed assignment. Continually assess the patient flow situations and notifying the appropriate persons when there are flow issues and delays in movement. Assure that director, nurse managers and directors are kept aware of related clinical and operational issues (discharges, admissions). Collaborate with Care Coordination staff on scheduled ambulance transfers and discharges.
