Key Highlights - Weekly pay: $2,802–$3,047 per week - Guaranteed hours: 40 per week - Contract duration: 13 weeks - Shift: Days - Start date: 08/27/2026 - Setting: Home healthcare - hospice Responsibilities - Prioritize start of care visits and manage scheduling to ensure timely initiation of home health services - Provide clinical supervision for Physical Therapy Assistants as assigned - Complete start of care visits with next-day reporting - Document clinical notes and other required documentation within 48 hours of each visit - Maintain timely, professional communication with supervisor, intake, and office; respond during normal business hours - Perform patient care within a home health framework and utilize the documented system (Kinnser) for ongoing records; equipment issued includes iPad and Verizon One Talk Requirements - Active California Physical Therapist license - At least 1 year of Home Health experience - Ability to perform Start of Care visits and associated documentation - Proficiency with the Kinnser documentation system or willingness to learn - Must be able to work in a home health setting and support territory needs as assigned Benefits and Support - Equipment provided: iPad, Verizon One Talk - Mileage reimbursement: $0.65.5/mile (from first to last patient) - Parking and toll reimbursement: Paid - Company car option available with a decreased bill rate - Training: 3 full days (one in-house and two in the field) - Travel assignment with guaranteed hours and a 13-week contract Find more opportunities Find additional openings at Healthforce Healthcare LLC by visiting <https://healthforce.applytojob.com/apply> PAY DISCLAIMER RULE Please note: Pay rates and guaranteed hours are estimates and may vary.
