CLASSIFICATION: Full-Time (Management Team)
EXEMPT/NONEXEMPT: Exempt
ANNUAL SALARY RANGE: $63,000-$68,000
GENERAL DESCRIPTION:
The Supervisor of Permanency ensures program compliance with all Federal, State, City and LSSNY requirements regarding foster home certification and recertification and Family Team Conferencing. Provides supervision of the Recruitment\Home Finder Specialists, Case Aide and Family Team Conference Coordinator. Ensures and maintains business processes that facilitate a diverse pool of foster homes that meet the needs of children and sibling groups, and effective agency facilitated conferences that promote permanency for children and youth in foster care.
MAJOR DUTIES AND RESPONSIBILITIES:
Oversee and maintain data reported in CONNECTIONS System
Recruit resource parents within assigned Community Districts
Train and provide orientation for prospective resource and adoptive parents
Assess resource parents’ abilities and motivations for becoming resource parents
Oversee and maintain various tracking systems (i.e., intake, orientation, etc.)
Assist with replacing disrupted placements
Receive and process intake calls from ACS and distribute to LSSNY Children’s Services staff
Confirm new placements
Process initial clothing allowance payments
Attend required meetings and trainings (ACS, OCFS etc.)
Prepare PAMS rebuttal
Verify compliance of foster homes with Federal, State and City Regulations
Process and monitor receipt of required certification and recertification documents and in Connections.
Conduct home visits to assess and compile documents required to complete thorough home studies, recertification, etc.
Follow up with families to obtain outstanding required documents.
Process applications, ensure all background clearances are conducted (i.e.: State Central Registry (SCR), Division of Criminal Justice Services (DCJS), etc.)
Create and maintain resource parents’ (active and inactive)/applicants’ records/files.
Maintain Resource Parent Master List
Ensure compliance with local, state, and Federal rules and regulations, including HIPAA.
Work on agency-wide projects as needed.
Develop and implement outreach strategies and materials to maximize agency presence in communities.
Act as agency liaison with community organizations, i.e., Community Partnership Initiative
Supervises the Family Team Conference Coordinator.
Work in collaboration with Case Planning teams to ensure that conferences are timely, maintain high quality standards, and are delivered using consistent protocols.
Ensures coverage, accuracy and timely conference documentation and dissemination.
Perform other duties as required by supervisor or needs of the program/ agency.

QualificationsEDUCATION:
Bachelor’s degree in Social Work, Human Services or related field
EXPERIENCE:
Minimum of two years’ relevant experience
Minimum of one year supervisory / management experience
MAPP\TRIPP trained.
SKILLS & ABILITIES:
Knowledgeable of working with foster care youth and children and families at risk
Understanding of social services theories and systems
Knowledge of Federal, state and city regulations
Excellent judgment and discretion; ability to make appropriate decisions to ensure the safety and well-being of families
Strong problem-solving and analytical thinking skills
Excellent communication skills (verbal and written)
Ability to establish a systematic course of action to accomplish specific
objectives, determine priorities and allocate time and resources effectively.
Ability to manage and direct staff effectively.
Ability to provide ongoing supervision & training.
Ability to take disciplinary action promptly, impartially, and consistently when necessary.
Ability to coach and develop staff by identifying strengths and areas needing development to promote longevity at LSSNY
Ability to work collaboratively with consumers and co-workers.
Ability to work independently and as part of a team
Experience with Connections, Binti, Microsoft Office
Ability to manage time effectively, multi-task and determine priorities.
Superior organizational skills
Ability to work flexible hours as needed.
On Call 24 hours
Ability to navigate through New York City using public transportation and/or valid New York State drivers license
Bilingual (English/Spanish), a plus
