Community Navigator - St. Rita's Medical Center

We are currently hiring a Community Navigator to support high-risk populations through care coordination and patient advocacy. This role partners with providers and community organisations to address health needs and improve patient outcomes. Ideal candidates will have significant community-based experience and bilingual proficiency in English and Spanish.
