Hospice Aide/ CNA

Hospice Aide Pay Range: $25.00 Schedule: Full-time Make a Meaningful Difference At Grace Hospice, we provide professional care with a personal touch. We are seeking a caring and dependable Hospice Aide to join our interdisciplinary team and help individuals receiving hospice care maintain comfort, dignity, and the best possible quality of life wherever they call home. Position Summary Under the supervision of the Hospice Aide Manager and RN Case Manager, the Hospice Aide provides personal care, comfort, and support to patients and families during an important time in their lives. Responsibilities include: Assisting with bathing, dressing, grooming, oral care, skin care, and incontinence care Assisting with ambulation, transfers, repositioning, and mobility Obtaining and documenting vital signs Providing light housekeeping and other support outlined in the patient’s care plan Completing required documentation accurately and on time Communicating changes in a patient’s condition to the appropriate clinical team member Maintaining patient confidentiality and following HIPAA requirements Treating patients, families, coworkers, and community partners with kindness and respect Qualifications Current Minnesota Nursing Assistant Certification and active status on the Minnesota Nursing Assistant Registry At least one year of experience as a nursing assistant or home health aide preferred Valid Minnesota driver’s license Daily access to a reliable and insured vehicle Ability to use or learn electronic charting on a tablet or mobile phone Strong communication and interpersonal skills A compassionate approach and a desire to support patients and families receiving hospice care Benefits Grace Hospice offers a comprehensive benefits package, including: Medical, dental, and vision insurance Short-term and long-term disability Basic and supplemental life and AD&D insurance Paid vacation, sick time, and holidays Healthcare and dependent care flexible spending accounts Grace Hospice is an equal opportunity and drug-free employer.
