About The Role

DescriptionAre you a compassionate and skilled Licensed Practical Nurse looking to make a meaningful difference in the lives of patients? The Jamestown Healing Clinic (JHC) is seeking an LPN to join our integrated care team and provide quality, patient-centered nursing care.

Position Highlights

Schedule: Monday–Friday, 5:30 AM–2:00 PM

JHC is an integrated care clinic offering primary care, dental, behavioral health, and substance use disorder treatment. In this role, you’ll work alongside an interdisciplinary team to support patients with substance use disorders and co-occurring conditions while helping promote stability, recovery, and continuity of care.

The LPN supports care coordination, medication administration, patient education, clinical procedures, and daily clinic operations within the scope of practice, provider direction, and clinic protocols.

What You’ll Do

Essential Functions

Maintain efficient progression for patient care throughout the day, including vital signs and rooming patients as necessary to assist care team

Conduct nursing procedures, therapies, tests and services as ordered by members of the medical staff and within scope of training

Support a safety-conscious work environment

Assist in assessing patient needs and support implementation of care plans under the direction of a licensed provider

Communicate appropriately with the person served regardless of their age, sex, gender, race, language or national origin

Support prioritization of patient care for all patient encounters in the order of face-to-face visits, telephone messages and written requests

Collaborate with the medical staff and Medical Assistant to keep provider schedules on time, support efficient patient flow, and complete patient care during face-to-face encounters

Support coordination of care across services and levels of care to ensure patient continuity.

Assist with coordination of care for patients with behavioral health conditions, substance use disorders, and co-occurring medical needs in alignment with clinic protocols

Provide general nursing care to patients including administration of medications, CLIA- waived testing, EKG and treatments in accordance with nursing and clinic standards.

Prepare patients for examinations. Screen patients for appropriate information. Instruct patients in collections of samples, specimens and tests

Provide patient education regarding medications, treatment plans, and health needs under the direction of a provider, with sensitivity to behavioral health and recovery needs

Support medication administration processes related to substance use disorder treatment, including medication-assisted treatment (MAT) following clinic protocols and scope of practice.

Collaborate with interdisciplinary team members including providers, behavioral health staff, and peer support to support patient engagement and continuity of care

Reinforce treatment plans and support patient adherence to medications and care.

Assist providers with procedures

Observe, record, and report patient’s condition including reactions to drugs and treatments.

Obtain and document patient history

Maintain and review patient records, charts and other pertinent information

Data entry and documentation of clinical information as needed

Respond to/refer incoming patient treatment related phone calls and instruct patient and family regarding medications and treatment instructions.

Practice of nursing within the scope of license and in accordance with the current standards of care and clinic protocols

Adhere to all JST and JHC policies and procedures, including standards for safety, and work practices as described in the JHC Occupational Exposure Control Plan and HIPAA

Perform additional duties as assigned.

What You’ll Bring

Requirements

  • Graduation from an accredited school of nursing
  • Valid Washington State license to practice as a Licensed Practical Nurse
  • First Aid/CPR certification
  • Knowledge of professional nursing theory, regulations and practices to evaluate and provide patient care
  • Knowledge of common safety hazards and precautions to maintain a safe work environment, including infection control and environmental safety
  • Skill in preparing and maintaining records and responding in writing as needed to correspondence regarding patients.
  • Demonstrate clinical competence in the provision of patient care
  • Ability to maintain quality nursing care standards.
  • Excellent organizational skills and the ability to prioritize multiple tasks
  • Effective time management, decision-making, and problem-solving skills
  • Exceptional interpersonal skills including effective verbal and written communication
  • Basic computer knowledge
  • Must be able to successfully pass criminal background check investigation
  • Valid Washington State Driver’s License: ability to travel locally and regionally assigned

Preferred Qualifications

  • Experience in integrated care, behavioral health, or substance use treatment settings
  • Experience working in interdisciplinary care teams
  • Experience working with patients with behavioral health or substance use disorders
  • Familiarity with trauma-informed care principles
  • Experience and knowledge of EPIC
  • American Indian/Alaska Native preferences apply

Physical Requirements/Working Conditions

Adequate hearing for stethoscope and telephone use, and to perform the position’s essential functions

Must be available to work any shift (to include all hours, weekends, and/or holidays as job duties require)

Must be able to lift, push and carry up to 30 lbs. unassisted

Physical ability to access all areas of the property applicable to your position

Must be able to withstand prolonged standing, sitting, walking, bending, climbing, squatting and twisting without restriction

Requires eye/hand coordination and manual dexterity; ability to distinguish letters, numbers and symbols

Must have the ability to reach at or below/above shoulder level on a continuous basis

Must be able to work in a fast-paced, busy, and somewhat stressful environment and maintain physical stamina, proper mental attitude while dealing effectively with customers, management, team members, and outside contacts while working under pressure and meeting deadlines
