About The Role

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Pay: $35.85 to $45.59 per hour

Location: Lafayette, CO

Schedule: Picking up shifts as-needed for PTO coverage. Monday-Friday daytime hours.

SUMMARY

We are hiring a Physical Therapist to support older adults through our PACE program. This role provides therapy services both in the PACE Center and in participants’ homes, helping individuals maintain strength, mobility, safety, and independence.

The PT works closely with an interdisciplinary team to evaluate participant needs, build treatment plans, provide hands-on care, and recommend equipment or home modifications when needed. This is a good fit for someone who enjoys geriatric care, variety in the workday, and being part of a team-based care model.

RESPONSIBILITIES

  • Complete physical therapy evaluations, reassessments, and treatment plans
  • Provide therapy focused on mobility, strength, balance, transfers, fall prevention, and overall function
  • Visit participants in their homes to assess safety needs and recommend equipment, adaptive devices, or modifications
  • Educate participants, families, and caregivers on exercises, safety techniques, and equipment use
  • Collaborate with the interdisciplinary team and participate in care planning
  • Document care, progress, and updates in the electronic medical record

QUALIFICATIONS

  • Current Physical Therapist license required
  • Degree in Physical Therapy required
  • BLS/CPR certification required or ability to obtain within 60 days
  • 2+ years of physical therapy experience preferred
  • Experience with older adults, home health, rehab, long-term care, or interdisciplinary care preferred
  • Strong communication, documentation, and clinical judgment skills
  • Comfortable working in both a center-based setting and participant homes
