Case Manager (Behavioral Health)

The focus of the Behavioral Health Case Manager is to provide the community with mental health prevention and early intervention. The Case Manager's role is integral to this goal and is expected to provide UHC patients with case management and access services to UHC Behavioral Health Clinic.
