Responsibilities Provide individualized skilled nursing care to pediatric patients in their homes Administer medications and treatments as prescribed Monitor patient conditions and report significant changes promptly Manage specialized medical equipment, including: Gastrostomy Tubes (G-Tubes) Tracheostomies (Trachs) Ventilators Collaborate with physicians, caregivers, and interdisciplinary healthcare teams Educate families on patient care procedures and safety measures Maintain accurate and timely clinical documentation in accordance with agency standards and regulatory requirements Qualifications Current New York State Licensed Practical Nurse (LPN) License – Required Minimum of one (1) year of LPN experience – Required Pediatric nursing experience – Strongly Preferred Fluent in English (verbal and written communication) BLS Certification – Preferred PALS Certification – Preferred Fully vaccinated against COVID-19 – Preferred Authorized to work in the United States (ITIN not accepted) What We Offer Competitive pay at $40.00 per hour Wound care visit opportunities at $80.00 per case Flexible scheduling options to fit your availability Medical, dental, and vision insurance 401(k) retirement plan Ongoing training and professional development opportunities Supportive clinical leadership and responsive administrative staff Meaningful, rewarding work that positively impacts children and families Why Join Affirmed Home Care?
