About The Role
Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Title: Licensed Practical Nurse
Schedule: Monday-Friday, 8am to 5pm
- Pay: $26.66 to $33.28 per year
- Sign-On Bonus: $6,000
JOB SUMMARY
The Licensed Practical Nurse (LPN) is a member of the PACE interdisciplinary team; provides nursing care to the participant and assists the Registered Nurse with the implementation of the Plan of Care. The LPN provides care for the participant in a manner that promotes an interdisciplinary team approach, respects participant’s goals for care, meets Medicare/Medicaid conditions of participation and accreditation requirements and reflects the mission and philosophy of TRU PACE. Participates in all areas of clinical services.
EDUCATION AND QUALIFICATIONS
- Must possess a current LPN license in the state of Colorado
- Minimum one year LPN experience – hospice or home health experience preferred
- Experience working with frail/elderly preferred
- Experience working with individuals with mental health needs desirable
- Bilingual in Spanish preferred
- Must successfully pass agency required background checks and drug screening
- IV certification and phlebotomy skills preferred
ESSENTIAL DUTIES
- Collects data regarding all body systems, signs and symptoms, deviations, acuity, vital signs, and responses in collaboration with the Registered Nurse and primary care physician,
- Incorporates participant preferences and goals within the plan of care
- Supports and respects the participant and caregiver’s unique diversity, including spiritual and cultural beliefs.
- Completes documentation in an accurate and timely manner as per agency standard and in compliance with regulatory standards
- Provides participant and caregiver education as appropriate; areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, and health and wellness through the lifespan
