Outpatient Pharmacist - Wellspan Pharmacy - Fairfield - PRN

Schedule PRN Pharmacist 20-35 hours weekly Day shift M-F General Summary Provides pharmaceuticals and drug information as needed for adequate patient care according to professional standards and practices.

Essential Functions: Compounds pharmaceutical preparations and/or labels and packages medications and pharmaceuticals. Assists with and/or supervises IV preparation and/or delivery. Advises patients and medical/nursing staff on use of various medications including dosage, side effects, composition, drug-to-drug interactions, drug-to-allergy interactions and drug-to-food interactions. Ensures proper controls and records of narcotics and other controlled substances as required by state and federal law. Monitors drug therapy to identify, prevent or resolve actual or potential drug therapy problems. Addresses problems with physicians and/or nurses when indicated. Provides clinical pharmacy services as needed. Administers immunizations, reviews patient profiles for immunization qualification and makes that recommendation to the patient. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s). Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise. Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. Assists with and/or supervises personal care/long term care medication delivery. Assists in preparing required reports by compiling information.

Qualifications Minimum Education: Bachelors Degree Pharmacy Bachelor's Degree Required Work Experience: Less than 1 year Relevant experience.
