<p>Your Role</p>
<p>- Manage your caseload: Perform comprehensive OASIS assessments, including Start of Care, Recertifications, and Discharges<br>- Provide skilled care: Administer medications, provide wound care (including wound vacs), and manage IV therapies<br>- Coordinate care: Serve as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to keep the plan of care on track<br>- Educate: Empower patients and families to manage their conditions safely at home<br>- Document: Complete accurate, timely clinical notes using AI-assisted tools</p>
<p>Requirements</p>
<p>- Current, active California RN license (required). Please include your license number with your application<br>- Minimum 1 year of professional nursing experience (required)<br>- Home health experience preferred. Strong hospital and SNF nurses are encouraged to apply, and we'll provide home health orientation and OASIS training<br>- Valid California driver's license and reliable transportation (required)<br>- Strong clinical judgment, organization, and the ability to work independently</p>
