</div></div><h3>Summary</h3>
<p>The Referral Coordinator is responsible for coordinating all rehab admissions to our Skilled Nursing Facilities.  Completes pre-admission screening, which may include on-site assessment, calculating cost of care and identifying barriers to safe discharge. Communicates with inter-disciplinary team members at the hospital and at our SNFs to ensure a smooth transition.  Knowledgeable on Medicare/Medicare Advantage/commercial insurance criteria for admission and obtains insurance authorizations when needed.  Serves as a resource to hospital partners to promote Bethesda and its services.</p>

<h3>Qualifications</h3>

<li>Bachelor's degree in related field preferred</li>
<li>Previous Health Care/Clinical experience</li>
<li>Previous hospital and/or post-acute setting experience</li>
<li>Marketing skills</li>
