What You'll Do

• Complete comprehensive patient assessments and develop individualized plans of care.
• Deliver skilled nursing services in accordance with physician orders and agency standards.
• Monitor patient conditions and coordinate care with physicians and the interdisciplinary team.
• Educate patients, caregivers, and families regarding disease management, medications, treatments, and self-care.
• Identify changes in patient status and intervene appropriately.
• Ensure accurate and timely clinical documentation.
• Participate in case conferences and care coordination activities.
• Promote patient safety, independence, and quality of life.

Qualifications

• Current Oregon Registered Nurse (RN) license.
• Graduate of an accredited nursing program.
• Current CPR certification or ability to obtain.
• Valid driver's license, reliable transportation, and automobile insurance.
• Strong assessment, communication, and critical thinking skills.
• Home Health experience preferred but not required.
