About the Role

Working within the Nottingham Health and Care Point (NHCP), the Prevention Officer plays a vital role supporting people to maintain their independence, wellbeing, and quality of life by providing face to face interventions, timely information, advice and guidance.
You will assess needs, identify risks, and work collaboratively with internal and external partners to connect people with appropriate services, community resources, and preventative solutions. The role requires a person-centred approach, ensuring individuals receive the right support at the right time to promote independence and prevent escalation to more intensive services.

About You
We are looking for a compassionate, motivated and citizen-focused individual who is passionate about supporting people to live independently.
You will have excellent communication and interpersonal skills, with the confidence to engage with people from diverse backgrounds and build effective relationships with colleagues, partners and stakeholders. You will be a strong problem solver, able to gather information, assess needs, and make informed decisions in a fast-paced environment.
The successful candidate will demonstrate empathy, resilience and professionalism, while maintaining a commitment to delivering high-quality customer service. You will be organised, adaptable and able to manage competing demands whilst maintaining accurate records and meeting performance expectations.
Experience of working with vulnerable adults, health and social care services, or within a customer-focused environment would be highly beneficial.
THE ESSENTIAL CRITERIA

Experience of working in a health, social care, housing, community or related setting.

The ability to gather, analyse and interpret information to assess needs and determine appropriate outcomes.

  • Excellent communication and interpersonal skills and an ability to build effective working relationships at all levels

Experience of working collaboratively with colleagues and partner agencies.

An understanding of safeguarding principles and the ability to recognise and respond to concerns appropriately.

A commitment to equality, diversity and inclusion, ensuring services are accessible and responsive to individual needs.
An Enhanced DBS check is required for this post.
