<p><strong><span data-contrast="none">Role Scope</span></strong><span data-contrast="none">: </span><span data-contrast="none">The New Participant Advocate plays a key role in creating a welcoming, participant-centered onboarding experience for individuals transitioning into Habitat Health's PACE program. This role builds trusted relationships with participants and caregivers while partnering with the Interdisciplinary Team (IDT) and cross-functional teams to facilitate a seamless transition into care. <li><span data-contrast="auto">Minimum of one (1) year of experience working with older adults, frail elderly populations, participant advocacy, care coordination, case management, client services, or a similar participant-facing role.</span><span data-ccp-props="{"335559738":200,"335559739":200}"> </span></li>

<p><strong><span data-contrast="auto">Compensation:  </span></strong><span data-contrast="auto">The anticipated base salary range for this position is <strong>$24 - $29/hour</strong></span><span data-contrast="auto">. This position is also eligible for a target annual <strong>bonus of 5%</strong></span><span data-contrast="auto"> and may be eligible for additional incentive compensation, as applicable.</span><span data-ccp-props="{"335559738":200,"335559739":200}"> </span></p>
<p><span data-contrast="auto">The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.</span><span data-ccp-props="{"335559738":200,"335559739":200}"> </span></p>
