Community Outreach Specialist - Home Care / Home Health

<p><strong>About the Role</strong></p>
<p data-start="438" data-end="651">The Community Outreach Specialist is responsible for driving referral growth and building strategic relationships for AbriCare’s personal home care services across North Kansas City, MO.</p>
<p data-start="653" data-end="1130">This role is ideal for someone with hands-on experience in personal home care or home health who understands how families access in-home support. You’ll serve as the face of AbriCare in the field, developing referral partnerships, educating the community on in-home care services, and consistently generating new client opportunities. This is a relationship-driven, field-based role focused on expanding access to personal care services and growing our presence in the market.  </p>
<p data-start="653" data-end="1130">Candidates must have prior experience in personal home care, home health, who understand how non-medical in-home care services are delivered and accessed. This role focuses on developing referral relationships, educating partners and families, and driving consistent growth through community-based outreach across North Kansas City.</p>
<p><strong>What You’ll Do</strong></p>

<li>Build Awareness & Connection
<ul>
<li>Represent AbriCare at senior events, health fairs, food pantries, and community gatherings.</li>
<li>Share our story and services with individuals, families, and organizations in the community.</li>
<li>Identify new opportunities for visibility through volunteering, sponsorships, and co-hosted events.</li>
</ul>
</li>
<li> Develop Partnerships
<ul>
<li>Build and maintain relationships with local referral sources: hospitals, social workers, AAAs, VA centers, and senior programs.</li>
<li>Collaborate with faith communities, housing authorities, and local nonprofits to connect with seniors in need.</li>
<li>Serve as a local resource - someone partners call when they have a family or senior who needs help.</li>
</ul>
</li>
<li>Drive Community Outreach Efforts
<ul>
<li>Coordinate and participate in AbriCare-led events such as bingo nights, caregiver workshops, or educational talks.</li>
<li>Support flyer and collateral distribution across assigned routes.</li>
<li>Keep our outreach database up to date with contact information and engagement notes</li>
</ul>
</li>
<li>Advocate for Access
<ul>
<li>Educate community members about Missouri Medicaid personal care services and how AbriCare can help them get started.</li>
<li>Distribute educational handouts and materials that build understanding and trust.</li>
<li>Be an active listener and voice for the community - bringing feedback to the team about what people need most. </li>
</ul>

</ul>
<p><strong>Required Qualifications</strong></p>
<ul>
<li>Minimum 1 year of experience in community outreach, marketing, healthcare liaison work, or related role.</li>
<li>High school diploma or GED required; associate or bachelor’s degree in communications, marketing, public health, or a related field preferred.</li>
<li>Demonstrated experience building relationships in community or healthcare settings.</li>
<li>Strong communication and presentation skills with comfort speaking to groups and individuals.</li>
<li>Proficiency in Microsoft Office and ability to track activities in CRM or similar systems.</li>
<li>Valid driver’s license, reliable transportation, and willingness to travel locally across St. Louis, Missouri and surrounding areas.</li>
</ul>
<p><strong>Preferred Qualifications</strong></p>
<ul>
<li>2+ years of experience in outreach, healthcare, or community development.</li>
<li>Experience working with older adults, people with disabilities, or Medicaid programs.</li>
<li>Familiarity with the Missouri aging and home care ecosystem, including AAAs, social service agencies, and community organizations.</li>
<li>Previous work with or for a home care, healthcare, or nonprofit organization.</li>
</ul>
