Nurse Navigator - Breast Health - MRMC

Responsibilities:

1. Job responsibilities include those listed in competencies document
2. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.
3. Functions as a part of the multidisciplinary team supporting patients and providers across the McLeod Health system related to breast health services.
4. Serves as resource for answering questions related to Breast health (as applicable)
5. Serves as community outreach liaison, providing education about cancer prevention, early detection and screening programs
6. Coordinates follow-up care for patients in conjunction with ordering providers and radiology.
7. Coordinates care with patients who are at high risk for breast cancer.
8. Supports programmatic development through maintaining strong, professional relationships with clinical care
9. Tracks and manages follow up for patients with identified needs as applicable
10. Serves as liaison between patients and clinical care providers, facilitating communication and coordination of care, facilitation of appointments, or other navigational services as needed.
11. Provides cross coverage to breast health nurse navigation team members as needed.
12. As requested, provides reports to administration.
13. Is knowledgeable of and adheres to American College of Surgeons accreditation standards.
14. Represents the organization in a positive and professional manner at all community events.
15. Completes annual training and annual educational requirements on time.

Qualifications/Training:
3 years of relevant clinical experience
Strong communication skills – verbal and written
Ability to work independently
Computer literacy in Epic, PACS, Microsoft applications
Licenses/Certifications/Registrations/Education:
Must possess a valid SC license- RN
Oncology Certified Nurse (OCN), Certified Breast Care Nurse (CBCN), Oncology Nurse Navigator-Certified Generalist (ONN-CG) or other specialty certification within 1 year of hire preferred.
Job Type: full-time; 80 hours biweekly
