Licenses & Certifications

• Board Certification or Board Eligible in Pediatrics. Board certification must be obtained within 5 years. Required
• Active Illinois medical license. Required
• Active DEA license or ability to apply for such license prior to or upon hire. Required
• Basic Life Support/CPR certification. Preferred
